Consents will NOT be handed out to all 9th grade students.  If for any reason, you do NOT want your child to take part in the class, please print and sign the consent.  The signed consent will need to be returned to the school prior to the scheduled class.    

If for any reason you do NOT want your child to take part in this class,  please sign this form and return it to school prior to the scheduled class.  If we do NOT have a signed form to not allow the child to take part in the class, we assume that we have your permission for your child to participate.
